In my view they are largely undesirable because they provide little end bearing, may not leave sufficient space for the inclusion of assistive devices in the prosthetic knee, and are perhaps better replaced by a long myoplasty amputation. The Gritti Stokes procedure in particular often gives rise to a number of complications most of these being due to the retention of the patella. Recent contributions suggest this operation is excellent at least in terms of wound healing. Unfortunately, it has persuaded a number of surgeons to employ the procedure for this reason alone. We are concerned in this contribution with the through-knee amputation or knee disarticulation. At the outset I should say that there seems to me very little validity in an argument about the relative merits of operative procedures at different levels of limb ablation except in the individual case. On this basis it will be assumed that an amputation can properly be done below-thigh level but for one reason or another cannot be done at below-knee level The knee-disarticulation operation has a long history of use extending back to the sixteenth century if not before (5,6,7,8). Like many surgical procedures its popularity has waxed and waned with changing surgical fashion and in this particular case with the prosthetic practice of the day. I believe the procedure has a stated place in our surgical armamentarium. In the first place it has a particular application during the growth period as it preserves the distal femoral epiphysis and thus the capacity for 80-90 percent of femoral growth. In this way we obtain a stump of maximum length when skeletal maturity is reached yet somewhat shorter than the

Pain and pain-related interference in adults with lower-limb amputation: comparison of knee-disarticulation, transtibial, and transfemoral surgical sites.

Pain and pain-related interference with physical function have not been thoroughly studied in individuals who have undergone knee-disarticulation amputations. The principal aim of this study was to determine whether individuals with knee-disarticulation amputations have worse pain and pain-related interference with physical function than do individuals with transtibial or transfemoral amputatio...

Knee disarticulation versus above-knee amputation.

If below-knee amputation is impossible, knee disarticulation should be considered before above-knee amputation, regardless of age and etiology. Knee disarticulation which leaves the femur and patella untoched offers many advantages. The surgical technique is simple and non-traumatic since no bone or muscle tissue is to be dissected. The thigh muscles are completely preserved and thus there is n...

Gait analysis in peripheral vascular insufficiency through-knee amputation.

Due to recent improvements in prosthetic limb componentry, through-the-knee (knee disarticulation) amputation has gained new interest for rehabilitation-minded amputation surgeons. Recent objective scientific studies from our gait analysis laboratory have described the walking characteristics of peripheral vascular insufficiency through-knee amputees, and have compared their function with simil...

Causes of death in a series of 4738 Finnish war amputees.

1 State Supervisor of Prosthetic Services, Ministry of Social Affairs, Helsinki, Finland. THE loss of a limb and its replacement by a prosthesis create conditions deviating from the normal. Walking is always more difficult. Loon (2) found that the energy consumption of amputees increases with the level of amputation. In the case of an above-knee amputation the effort of walking is greater than ...

Evaluation of Gait Performance of a Hemipelvectomy Amputation Walking with a Canadian Prosthesis

Background. Hemipelvectomy amputation is a surgical procedure in which lower limb and a portion of pelvic are removed. There are a few studies in the literature regarding the performance of subjects with hip disarticulation during walking. However, there is no study on gait analysis of hemipelvectomy subject. Therefore, the aim of this paper was to evaluate the gait and stability of subject wit...
